This brief survey asks you to provide contact information for:
1. The person in your department/hospital who is responsible for signing data use agreements (DUA) or memorandums of understanding (MOU) for data sharing.
2. Your hospital's AIM Maternal Equity Bundle Champions.

Note: If your hospital has previously participated in an AIM  Bundle, then you do not need to re-sign a data sharing agreement. Please email PNQINAdmin@pnqinma.org if you have questions!
